Celebrating 30 Years of Health Equity!

The Center for Well-Being is thrilled to announce our annual Celebration of Dreams on
October 19
th 2024.

For over 30 years, we have radically improved quality of life in California through evidence-based programs, serving 25,000 community members annually. Proceeds from this event help offset the cost of our clinical and community-based programs, ensuring care for all, regardless of ability to pay. Join us in celebrating our impact and making Sonoma County healthier and more equitable for all!

¡Viva La Vida!

Join us for our 30th anniversary celebration! This will be an evening to commemorate all we have achieved together to advance a more equitable and just community for all. We'll enjoy excellent food, wine, music, and dancing - but most importantly, we'll be in community together. We'll hear heart warming stories from the youth, families, and seniors that we've supported to live their best lives. This is truly a time to celebrate all that we've achieved together! ¡Viva La Vida!

How Your Dollars Help

All proceeds provide FREE access to comprehensive care coordination and resource connection for low-income Sonoma County children, families, and seniors facing significant health disparities. These individuals often experience gaps in basic needs such as food, housing, and financial resources. All of our clients have one or more chronic health conditions, including 22% who are children aged 8 to 17 years old. They all receive evidence-based clinical services that have been proven to radically improve health outcomes. Our services are offered in both English and Spanish. Our client demographics include 64% Latinx, 27% white non-Hispanic, and 9% identifying as other.

We know that health and well-being is more than just the absence of disease – it’s having access to the tools, knowledge, and resources to be well, and to be seen, heard, and supported.
