Health Equity Education & Outreach
For over 30 years, we have ensured that Sonoma County’s most vulnerable and marginalized community members have access to the resources, education, and support they need to thrive. Our health equity campaigns are designed and delivered by Community Health Workers | Promotoras de Salud (CHW/Ps)—trusted messengers who provide culturally responsive health information and connect residents to care. This community-driven approach strengthens health equity, reduces costly barriers to care, and advances lasting social justice across our region.
Examples of our CHW-led education campaigns include:
- Blood Pressure Awareness: Promoting early detection and management of hypertension to reduce cardiovascular risk.
- STD and HIV Prevention: Providing culturally tailored education on safer sex practices and access to testing and treatment.
- Tobacco Prevention and Cessation: Educating residents on the risks of tobacco use and supporting quitting efforts.
- Census Participation Awareness: Ensuring historically undercounted communities are informed and represented.
- Nutrition and Chronic Disease Education: Teaching families about healthy eating, diabetes management, and strategies for long-term wellness.
Connecting Seniors to Essential Resources
Juntos de la Mano, or "Together Hand in Hand," is a community resource program led by Community Health Workers (CHWs) at the Center. This initiative is designed to support Sonoma County's senior residents by connecting them to vital resources and services, such as food, healthcare, mental health, housing, and senior-specific support.
Led by trusted CHWs, Juntos de la Mano empowers seniors and caregivers with education, support, and easy access to services. Our CHWs bridge cultural and language gaps to ensure everyone receives the care they need. By creating meaningful connections, Juntos de la Mano is building a more equitable, accessible network of care for our community.

County-Wide Vaccine Equity Campaign
Funded by the Sonoma County Board of Supervisors, the Northern California Center for Well-Being is leading a county-wide vaccine equity campaign designed to strengthen the resiliency of historically underserved communities. Through this initiative, our Community Health Workers | Promotoras de Salud (CHW/Ps) provide culturally relevant and linguistically appropriate outreach, education, and connections to vital resources.
We are proud to partner with La Familia Sana, La Luz, Healthy Petaluma, Roseland CBI y Equidad, Grupo de Mujeres Unidas, and the West Russian River Promotoras, ensuring that all five regions of Sonoma County are reached with trusted, community-driven support.
To remove barriers to care, we have also partnered with Providence St. Joseph Health to deliver vaccines directly to where people live, work, and gather.
This comprehensive, collaborative approach reduces health disparities, expands vaccine access, and protects our most vulnerable neighbors from COVID-19, flu, RSV, and other communicable diseases—advancing health equity across Sonoma County.
